Personalized Report: To receive the personalized LivAbility^sm Report, you must first complete and submit the questionnaire. Using your answers, LivAbility^sm program will draw upon a database of products and ideas to prepare a customized report - just for you. The questionnaire requires about 15 minutes to complete.

1) Authorized Zip Code

Ease Metro SAIL has provided funding that made placement of the LivAbility^sm Questionnaire on this web site possible.  If you live in Washington, Ramsey or Dakota counties in Minnesota, you have an authorized zip code.

2) Valid Membership Number

Organizations may purchase memberships to access LivAbility^sm for their members.  Contact mchristenson@lifease.com about receiving membership numbers.

3) Credit Card

The cost is $19.95.  This is a secure site; you can submit your credit card information with complete confidence.  If you prefer, however, you may order by calling 612-312-1853 or by fax 612-706-9617.  We also accept personal checks and corporate purchase orders.
